You must fill out the pdf application and email the attachment to lincolnparkgoodfellows@gmail.com

   Please enter all requested information in the PDF before you attach and send the email.

To be considered for assistance, You must supply copies of the following requested documents:

• Current picture ID: Valid Drivers License and/or State ID
• Birth Certificate for each child you are requesting assistance for
• Current Mortgage Statement or Current Rental Agreement
• Current Utility Bill

Lincoln Park Goodfellows understands that you may have limited information regarding a family you are requesting/needing assistance for. However you must provide a good telephone number for Goodfellows to contact.
Submission of this form does not guarantee approval for assistance. 

Please leave a contact number if you submit a families name for assistance, as sometimes numbers get changed.
